Practice Days & Time:
  Monday, Tuesday, Wednesday, and Thursday evenings from 6:45-8:15 pm through mid November.

 
Mandatory Concussion Training at Porter: Monday, September 26 @
7:00 pm (except for athletes who already attended a session at another Middle School).

Mandatory Parent & Player Meeting: Monday, September 26 @
8:00 pm (immediately following the concussion training session).  If you attended a Concussion Training Session at another MS school, just come at 8:00 pm for the Parent & Player Meeting.

First Practice: Tuesday, September 27 @
6:45-8:15 pm

Equipment Needed:  Volleyball (optional), water bottle, v-ball knee pads, athletic shoes & practice clothes

 

Games:  Only players who have a current sports physical (after May 1, 2011) on file and have transportation to/from the games will be eligible to play in these games.

Wed. Oct 5 away at Holy Family - 4:00 warm-up; 4:30 game

Wed. Oct. 19 home vs. Saint William of York - 4:30 warm-up; 5:00 game

Thurs. Oct. 20 home vs. Aquinas JV Team - 3:45 warm-up; 4:15 game

Mon. Oct. 24 away at St. Francis - 4:30 warm-up; 5:00 game

Thurs. Oct. 27 away at Aquinas JV Team - 3:45 warm-up; 4:15 game

 

Forms:  All the forms (except the Sports Physical page) have to be completed and turned in to Mrs. Misencik before 2:00pm on Monday 9/26/11, so the roster can be finalized before the first practice night.  Mrs. Misencik is in charge of the paperwork, not Coach Dettmann.  Students will not be permitted to participate in practice until the player & parent attends a mandatory Concussion Training Session and all the forms (except the Sports Physical) are completed and returned.  If a player wants to be eligible to play in the first game, the Sports Physical form needs to be turned in to Mrs. Misencik by Tuesday, October 4, 2011 The forms can be found in the "Team Files" section below.  There are 3 different files you need to download.  Don't forget to print out and attach the Soccer Participant Checklist to your forms.

Sports Physicals:  must be current, as of this school year (dated after
May 1, 2011). 

Local Urgent Care, NextCare, and CVS Minute Clinics will often do sports physicals without an appointment or insurance card (please call them to verify this and the price.)  Make sure you take the PWCS form with you and complete the patient "Medical History" section in advance.  Remember, if a player wants to be eligible to play in the first game, the Sports Physical form needs to be turned in to Mrs. Misencik by Tuesday, October 4, 2011.

Mandatory Concussion Training for ALL PWCS Athletes and Parents

All PWCS High Schools and Middle Schools are in the process of scheduling Concussion Presentations for parents and students according to PWCS Policy 759.  Please check individual school websites periodically for updated sessions and times.

As there are sufficient differences in the way concussion will be handled at the middle school and high school levels, a parent having children in both middle and high schools, will need to attend both presentations.

The meeting is mandatory for ALL students and their parents who will be trying out and/or playing any sport (Porter Intramurals or Travel Team) during the 2011-12 school year.  

A Porter student cannot try-out for or participate in a sport unless they, along with a parent, attend one of the information sessions.  Your student will also need an up to date physical in order to try-out for a Travel Sports Team at Porter.  

All the PWC Middle Schools will hold the mandatory concussion trainings. You are welcome to attend the meeting at any location.

More dates will be available for the winter and spring seasons, but it is recommended you attend a class at your earliest convenience!

Please feel free to check the website of a PWCS Middle School near you to find alternative dates.

Please call the MS above to confirm these dates/time prior to attending the meeting in case there is a last minute schedule change.

Sports Physicals

In addition to Intramural Sports (no Sports Physical required), Porter sometimes offers a travel team tryout for Middle School Boys Soccer, Boys Baseball, Girls Volleyball, and Boys & Girls Basketball.  If you think your child might want to tryout for one of these sports, please get a Sports Physical on file with Mrs. Misencik at your earliest convenience.  Just like other Middle Schools, players are not allowed to tryout unless a sports physical is on file.  Sport Physicals are only good for one school year.  If you got one is the spring, it is good from May 1, 2011 through June 30, 2012. 

A Sports Physical is not needed for Flag Football since it is an in-house Intramural sporting opportunity.

This Emergency Card must be completed by a Parent and turned in with Pages 1, 2 & 4 (below). Players must have the proper forms on file with Mrs. Misencik in order to begin practicing with the Flag Football, Golf, Soccer, or Volleyball Team.

Only get Page 3 completed (Sports Physical) if you plan on trying out for a Travel Team. Intramural players do not need a sports physical since they will not compete against students from other schools.

These forms must be turned in to Mrs. Misencik before trying out for a sports team.

Sports Physical Info: Parents need to complete Page 2 (Medical History completed by the Parent) and take it with you to the appointment, as the Doctor will review the Medical History from the parent, before completing the Physical on Page 3.

NextCare and the Lake Ridge Urgent Care take walk-in appointments and the fee is between $30-$45. It is not necessary to present an Insurance card. Please call ahead to verify this information. Follow this link to download a coupon for a $25 sports physical at
